LIST OF BLANK FORMS PERTAINING TO THE QUARTERMASTER CORPS.

The following list of blank forms used by the Quartermaster Corps is published for the information and guidance of all concerned. It consists of War Department standard forms, Quartermaster Corps forms, Government stock transportation voucher forms, and compensation act forms prescribed by the Secretary of Labor for use in administering the Government workmen's compensation act of May 30, 1908 (35 Stat., 556). This list replaces the list published in Circular No. 12, O. Q. M. G., June 8, 1916.

6 Oath of office..

The combined letter form on which the au-
thority of the Secretary of War is sought,
through the Quartermaster General of the
Army, for publishing advertisements, and
the approval of the request.

Space is provided on the form for listing the
publications considered by the requesting
officer to be the best medium for securing
publicity for the purposes of the Government.
In case special authority is desired, a copy of
the proposed advertisement must accom-
pany the application.

This form is also used when requesting author-
ity to readvertise in the cases provided for by
Army regulations.

Form of affidavit required by law to be fur-
nished by the proprietors or publishers of
newspapers. The request for the affidavit
contained in the form is forwarded to the
publisher by the officer concerned. It is
signed by the assistant and chief clerk, War
Department, or by the department com-
mander if the case originates in his jurisdic-
tion, and when received back from the pub-
lisher completed it is returned to the office
which requested the sworn statement.
Form of oath prescribed by section 1757 of the
Revised Statutes, as amended by the act of
June 6, 1898 (30 Stat., 432), to be taken and
subscribed to by every person elected or ap-
pointed to any office of honor or trust under
the Government of the United States before
entering upon the duties of his office.
An employee whose compensation is fixed by
the head of an executive department is not
required to take a new oath of office when his
compensation is increased or diminished.

and certification.

Form for reporting all persons who may be re-
instated during the month.

Form for reporting all transfers within the War
Department during the month. Its use is
limited to transfers from one bureau of the
War Department to another, or from one
position to another in the same bureau or
service, if not promotion or reduction; or
from the field service to the service of the
same department in Washington. If such
transfers include promotion or reduction, re-
port must also be made on W. D. Form 16g.
For reporting all persons transferred during the
month to and from the department. Trans-
fers from the department must not be re-
ported under "Separations," W. D. Form
16k.

Form for reporting all persons who have been
promoted or reduced during the month.
Promotions made on certification or special
approval of the Civil Service Commission
should show that fact under a subheading.
Form for reporting separations by removals,
resignations, and deaths during the month.
In the case of a probationer, date of appoint-
ment must also be given. This form is used
only for the classified service.
Form for reporting appointments to excepted
positions made during the month. These
positions are excepted from competitive ex-
amination or registration for appointment.
Form for reporting separations from excepted
positions during the month. The cause of
separation must be stated.

Form for reporting appointments to the unclas-
sified service during the month. In the case
of appointees who perform manual labor, the
kind of manual labor must be stated. In the
case of other appointees the nature of the du-
ties performed must be stated.

Form for reporting separations from the un-
classified service during the month. The
cause of separation must be stated.

Form for reporting promotions, transfers, or
changes in duty in the unclassified service
during the month. The duties to which at
present assigned must be stated.

Forms 9, 9a, 9b, 96.1, 9c, and 9d will not be used in offices for which the flat filing system for spondence and records has been authorized.
